Essential Functions
- Audit and approve EMR tasks including, but not limited to, admission, plan of care, recertification and discharges including documentation of OASIS assessment.
- Perform compliance audits and accreditation audits
- Establish priorities for quality and performance improvement activities and develop performance improvement projects and actions plans to support the delivery of safe, highly reliable, quality patient care services.
- Communicate success of outcomes by preparing/providing accurate and clear status reports for leadership to include analysis, recommendations, updates, and modifications.
- Proactively researches Home Health regulatory sites to identify potential quality measures to forecast future needs and assure compliance within industry standards.
- Participates on various committees related to quality improvement and patient safety. Includes oversight of the QAPI program and QAPI committee.
- Participates in Root Cause Analysis cases as needed.
- Plan and provide annual education and skills day
- Provide Aide monthly education
- Provide Aide onboarding and annual competencies
- Oversee nursing and therapy orientation
- Maintain skills to perform skilled nursing visits when needed
- Other duties as assigned
Skill Requirements
- Must exhibit a comprehensive understanding of quality improvement methodologies, theory, and practice.
- Thorough understanding of Home Health state, national and accreditation organization (ACHC) regulatory standards related to patient safety and quality assurance.
- Must have thorough understanding of home health documentation requirements including admission, plan of care, recertification and discharge to meet compliance standards
- Knowledge of OASIS and maintain competency with future OASIS changes
- Effective oral, written and presentation skills. Proficient in word, excel, and PowerPoint, research and data management skills.
Education Requirements
Registered Nurse (RN) with a Bachelor’s Degree, required, Master’s Degree preferred.
Licensure and Certifications
License: RN License valid in the state of Virginia
Experience Requirements
- Minimum 3 years of Home Health experience
- Quality experience preferred
- Education Experience preferred
- Home Health Leadership experience preferred
